As we say here, not fer nuttin' but that pistol is designed so that it can fire the Colt .45 and a .410 shotgun shell, both in slug and buckshot forms.
A .410 slug at close range makes a big freakin' hole. With the slug passing thru his thigh he's damm lucky he didn't hit his femoral artery because he would have bled out in minutes.

somebody should invent a mechanism that would keep a gun from firing until you want it to. maybe call it a safety.

Note that it comes with a safety of sorts locking the trigger. Also it has a 9.5 pound trigger pull in DA, 4.75 pounds in SA mode. Hard to imagine a accidental discharge. I'll bet he was driving along playing Bronco Billy twirling it when it went off.
