Those stupid wheels with the wrong offset should have killed that person. The one in the truck not the Joey Chitwood driver.

The wheel bearing came apart. Can see the rotor is still attached to the wheel. Find it hard to believe it wasn't driving badly and making a lot of noise. But the cupped tires from misalignment probably drowned it out.
 
This was a couple of miles from my house. As @KTMLew01 said, the rotor was still attached to the wheel, so it wasn’t lug nuts. Either the wheel bearing failed, the castle nut wasn’t pinned or the spindle broke. I would say the bearings would be the most likely reason.
You can’t tell me that bearing wasn’t making a ton of noise for quite a while.

That is the craziest thing I've ever seen. The fact that he only suffered minor injuries is just nuts. It looks like something that came out of a movie.
